Prevalence and Factors Associated with Uncontrolled Hypertension: A Cross-sectional Study

Uncontrolled hypertension remains a major public health concern because it increases the risk of cardiovascular morbidity and mortality, particularly in low- and middle-income settings. This study aimed to estimate the prevalence of uncontrolled hypertension and identify factors associated with poor blood pressure control among adults with diagnosed hypertension receiving follow-up care in Rasau Jaya Village, Kubu Raya Regency, Indonesia. A cross-sectional study was conducted among 252 adult patients with diagnosed hypertension. Data were collected using a structured questionnaire and medical record review, and blood pressure was measured using standardized procedures. Uncontrolled hypertension was defined as systolic blood pressure ≥140 mmHg and/or diastolic blood pressure ≥90 mmHg. Bivariate analyses and multivariable regression were performed to identify factors associated with uncontrolled hypertension. The prevalence of uncontrolled hypertension was 59.5%. In multivariable analysis, family history of hypertension and hypertension duration of five years or longer remained significantly associated with uncontrolled blood pressure. Participants with a family history of hypertension had a higher likelihood of uncontrolled hypertension (APR = 2.34; 95% CI: 1.36-4.04). Similarly, participants with a hypertension duration of five years or longer were more likely to have uncontrolled blood pressure (APR = 2.18; 95% CI: 1.23-3.86). These findings indicate that uncontrolled hypertension is common among patients receiving follow-up care in this setting. Family history of hypertension and longer disease duration were key determinants of poor blood pressure control. Strengthening routine blood pressure monitoring are essential to improve hypertension control and reduce cardiovascular risk.
